Jeremiah was born Tuesday, September 09, 1980 in Columbus, OH to Jeffery and Valerie Frazier.
He is survived by both parents, Signifiant other, Jessica Adkins, two daughters, Madison and Violet, a sister, Christy Frazier, two nephews, Anthony & Matthew. grandparents; Pearl and Hazel Murray, Taylor and Mary Frazier.
And a host of aunts, uncles, cousins, and numerous family members that dearly loved Jeremiah.
